Submitting an Incident Report
Incident reports document security events, accidents, observations, and unusual activity at your site. Filing clear, detailed reports helps your team respond effectively, maintain records for legal purposes, and improve site security over time.
When to File an Incident Report

You should file a report whenever you observe or respond to:
- Security incidents — unauthorized access, trespassing, theft, vandalism.
- Accidents — injuries, falls, vehicle collisions on site.
- Suspicious activity — unusual behavior, unidentified persons, abandoned packages.
- Property damage — broken windows, graffiti, water leaks, structural damage.
- Safety hazards — spills, exposed wiring, blocked fire exits.
- Any event your supervisor would want documented.
For immediate emergencies where you need help right now, use the Emergency SOS Button instead.
Accessing Incident Reports
- Tap the More tab at the bottom of the screen.
- Tap "Incident Reports" (Alert Triangle icon, orange).
- You'll see two tabs:
- Report — create a new incident report.
- History — view your previously submitted reports.
Creating a New Report
- Tap the Report tab.
- Choose the report category from the cards displayed:
- Incident Reports — for security and safety events.
- Daily Activity — for routine shift activity logs.
- Accident — for injuries and vehicle incidents.
- Other categories — your admin may have configured additional types.
- Fill in the incident report form:
Required Fields
- Type — select the specific incident category from the dropdown (e.g., Theft, Trespassing, Fire Alarm, Medical).
- Title — a short, clear summary of what happened (e.g., "Unauthorized vehicle in loading dock").
- Description — a detailed explanation of the event. Include who was involved, what happened, when it occurred, and what actions you took.
Adding Photos
Photos provide crucial visual evidence. To add them:
- Tap "Add Photos" (Camera icon).
- Choose:
- "Take Photo" — opens your camera to capture a new image.
- "Upload from Library" — select an existing photo from your phone.
- You can add multiple photos — capture different angles, damage details, and the wider scene.
Location
- Your GPS location is captured automatically when you start the report.
- Tap "Use Current Location" to refresh your coordinates if you've moved since starting.
- The coordinates are displayed so you can verify accuracy.
Witnesses and Involved Persons
If there were witnesses or people involved in the incident:
- Tap "Add Person."
- Enter their name, role (witness, suspect, victim), and contact information.
- Add multiple people as needed — each person gets their own entry.
Attachments
- Tap "Upload Document" to attach PDFs, scanned documents, or other supporting files.
Digital Signature
- A signature pad appears at the bottom of the report form.
- Sign with your finger to authenticate the report. This confirms you are the reporting officer.
Priority Level
Set the urgency of the report:
- Low — informational, no immediate action required.
- Normal — standard incident, follow-up at regular pace.
- High — urgent situation, requires prompt attention from management.
Notify Supervisors
- Check the boxes next to the supervisors who should be alerted immediately about this incident.
- Selected supervisors will receive a push notification.
Voice Notes
When typing isn't practical (e.g., you're in a hurry or on the move):
- Tap and hold the microphone button to begin recording.
- Speak clearly and describe what happened.
- Release the button to stop recording.
- The voice note is attached to the report automatically.
Saving and Submitting
You have three options when you're done:
- "Submit Report" — sends the report immediately to your supervisors and the admin dashboard. Once submitted, it cannot be undone or edited.
- "Save as Draft" — saves your progress locally so you can come back and finish later. Great if you need to gather more information first.
- "Cancel" — discards the report entirely. You'll be asked to confirm before anything is deleted.
Viewing Your Report History
- Tap the History tab on the Incident Reports screen.
- Browse your past reports in chronological order.
- Each report shows its status:
- Submitted — received but not yet reviewed.
- Reviewed — a supervisor has read the report.
- Closed — the incident has been resolved and the case is closed.
- Tap any report to view the full details.
Tips
- Report immediately — file the report as soon as possible after the event while details are fresh in your memory.
- Add photos from multiple angles — visual evidence is often the most valuable part of an incident report.
- Be specific and factual — include exact times, physical descriptions, vehicle plate numbers, and locations. Stick to facts, not assumptions.
- Use voice notes for quick capture — if you're in the middle of responding to an incident, record a voice note first and add written details later.
- Save as draft if unsure — you can always come back to finish a report. It's better to start capturing details immediately than to wait until you have everything.
- Don't duplicate reports — check your History tab before filing to make sure someone else hasn't already reported the same incident.
- Include what actions you took — did you call the police? Secure the area? Contact the building manager? Document your response.
Troubleshooting
Report not submitting
- Check your internet connection — try switching between Wi-Fi and cellular data.
- Save as draft and try submitting later when you have a better connection.
- If the problem persists, restart the app and try again.
Camera not working
- Go to your phone's Settings > Attlock > Camera and make sure permission is granted.
- If the camera opens but photos are blurry, clean your lens and make sure there's adequate lighting.
Voice recording not working
- Make sure microphone permissions are enabled for the Attlock app.
- Check that your phone's microphone isn't blocked by a case or debris.
